Background
This includes the patients review of systems, the physical exam, PMH, diagnostic information, medications. Please list the forceful features in the presentation case study that assisted you to decide on diagnosis and differentials and prescribe the current pharmaceutical therapy. Be specific.

Assessment
This includes your diagnosis and differential diagnosis. As these cases are usually the more troublesome or complex you will have differential diagnosis.
Response
This includes what you did for the patient, medications include dose, route and duration, other testing, referral with treatment plans, and any for alternative therapy that can include alternative herbals, vitamins or treatments. What kind of teaching, comfort care, palliative care, nutritional, would you provide and when will the patient follow up with you.
