As we have learned, the purpose of an intake requires the substance abuse counselor to use effective communication skills as well as have an understanding of the components for this type of assessment. These skills include the ability to hear and understand verbal and nonverbal messages while listening responsively to both kinds of messages within an empathic context. The completion of an intake assessment also requires the substance abuse counselor to evaluate the client’s concerns and keep their sensitive information confidential.
Use the attached Intake Assessment form to conduct the interview with your prospective client. The completed form must be uploaded via the drop box. Remember that all information must be keep confidential, and all parts of the intake must be completed, even is the answer is “N/A” — Not Applicable.
