Students will prepare the presentation using the following scenario and the required outline. You will need to complete a literature search to find a best practice scholarly article for patient falls. The evidence from the literature will assist you in development of your presentation
Quality Improvement Scenario
You are a nurse leader in the medical/surgical division of a large teaching hospital. Despite standard yearly education, patient falls over the last 6 months have been 4.52 falls/1000 patient days, when the organizational goal is 3.42 falls/1000 days.
Quality Improvement Outline
1. Overview of the Problem:
State the problem.
2. Aim Statement:
Write an Aim Statement that is specific, measurable, attainable, reasonable, and has a time frame.
3. Choosing Measures:
Use the family of measures to identify each measure:
o Outcome Measure: Where are you ultimately trying to go?
o Process Measures: Are you doing the right things to get there?
o Balancing Measures: Are the changes you are making to one part of the system causing problems in other parts of the system?
4. Developing Changes: What change can you make that will result in improvement?
Five useful ways to develop changes are: critical thinking, benchmarking, using technology, creative thinking, and change concepts.
5. Testing Changes:
Plan: Plan the test or observation, including a plan for collecting data.
Do: Try out the test on a small scale.
Study: Set aside time to analyze the data and study the results
Act: Refine the change, based on what was learned from the test.
6. Implement Changes:
How will you hardwire or make the change permanent, through tactics such as:
Documentation
Training
Addressing supply issues
Assigning day to day ownership for the new process/standard
