Write a paper on Translating Evidence into Practice.

The background question

The model you will use to guide your project and change practice, including a brief rationale for that choice
The Search Tracker that includes the PICOT question and delineates the steps of your search process (included in an appendix)
Evaluation and Synthesis Tables based on the pared-down search, with a narrative that explains the results and includes:
What the evidence tells you about best practices for this topic
How the body of evidence will help you design your practice improvement project
Outcomes that you will measure to determine whether a change has occurred, their sources (electronic health record [EHR], deaths, etc.), and how the data will be evaluated (this may be in the form of a grid in the appendix)
The change model you will use to motivate stakeholders with a brief rationale.

Page -1. you appraised quantitative evidence from a reference list that was provided. You then set up an Evaluation Table for your individual PICOT question. In this week, you will add to that table by adding the critical appraisal of a qualitative study. Use the reference list provided in the Submissions Area and critically appraise the qualitative study. Compile the following:

1A. A copy of the RCA form for the qualitative study
2A. A completed Evaluation Table that includes the quantitative and qualitative studies

Page- 2. Working Ahead—Qualitative Evidence
Nurses are typically less familiar with qualitative studies than quantitative. When doing a thorough literature review to find external evidence, you must consider all points of view. This assignment will provide you with an opportunity to read and appraise a qualitative study and objectively determine its worth to inform your PICOT question. Tasks:- In this assignment, you will use the same clinical scenario from this week’s discussion. Click here to review it. On the basis of your review of the scenario, in a 1-page APA-formatted paper, respond to the following:

2A. What is your professional judgment in prescribing a weight loss drug for a patient who is not following dietary recommendations or exercising?

Page-3. Analyzing and Synthesizing the Body of Evidence – The Synthesis Table Template has been provided that includes several different types of tables. Choose the tables that are most pertinent to this project. Review the PICOT question you wrote in Submissions Area and the Evaluation Table you started in Submissions Area with the quantitative evidence and complete it by adding the qualitative evidence in Week 5 . Then:

3A. Analyze the contents of the table. Which variables (column headings) are the most important to answering the PICOT question? Why?
3B. Create the appropriate Synthesis Table (or tables) and turn it in for this assignment along with a 1-page paper providing.
3C.A reason for including these variables in the Synthesis Table.
3D. A rationale that explains how these variables are most appropriate for answering your PICOT question.

4. Thinking Ahead—Outcome Measures – Your practice setting may provide options for collecting data from various sources, such as EHR, admissions and discharges, and morbidity and mortality reports. If not, you will need to determine what data you need, when it should be collected, who will collect it, and how you will collect it. In addition, you will need to determine how it will be analyzed.

4A. Review the Synthesis Table you created in the Submissions Area for your project and compare the result with the desired outcome from your PICOT question.
4B. Identify outcome measures that you will monitor as indicators of practice change, deciding how the data will be collected (pre- and posttests of participants, EHR, admission and discharge data, etc.) and analyzed.

4C. Use the Outcome Table Template to organize your outcomes. Fill out the table to include variables of interest, data needed, frequency of data collection, methods of data collection, who will collect, and a description of the analysis conducted.
4D. List your outcomes in the far left column, filling in the other columns for each outcome.

4E. Any statistical manipulation that may be necessary to help analyze the outcomes, with a rationale as to why this method is appropriate.
4F. A brief statement about how you will motivate your coworkers or team to collect this data.
4G. You will turn in both the table and a narrative for this assignment.

5. Translating Evidence into Practice
develop a practice improvement project. Beginning with the burning question, you searched the literature to determine the state of the science on the topic. After analyzing the evidence, the best evidence became the intervention you would implement and evaluate as you translate research into practice.
Compile the five sections of topics above including the following elements

5A. The Search Tracker that includes the PICOT question and delineates the steps of your search process (included in an appendix)
5B. Evaluation and Synthesis Tables based on the pared-down search, with a narrative that explains the results and includes:
5C. What the evidence tells you about best practices for this topic
5D. How the body of evidence will help you design your practice improvement project
Outcomes that you will measure to determine whether change has occurred, their sources (electronic health record [EHR], deaths, etc.), and how the data will be evaluated (this may be in the form of a grid in the appendix)
5E. The change model you will use to motivate stakeholders with a brief rationale.
