The information must be expanded upon to answer the following: 1.)Identifying information and a brief history In 3-4 paragraphs give a brief psychosocial-spiritual assessment of your client highlighting keys areas including: presenting issue, treatment history, social history, trauma/abuse/neglect history, substance use history, and relationship history. 2.)Attachment History (Ainsworth and Bowlby) Discuss your client’s attachment history. How does your client’s attachment style impact their ability to form and maintain relationships? 3.)Neurobiology Describe and discuss the main neurobiological processes your client uses during your sessions. Hypothesize how your client has formed these processes and how they may help or hinder the therapeutic process. Be sure to discuss the neurobiological process of your client’s affect regulation abilities, memory capacities, and ability to use higher order brain function. 4.)Diagnoses & Treatment Using the DMS V (can be found online), formulate a primary and secondary diagnosis/diagnoses of your client from an attachment informed and neurobiologically informed perspective. This may differ from the current working diagnosis. Support your diagnosis with a clinical formation. Create a treatment plan supporting the attachment and neurobiological needs of your client.
