Page 1: Introduce reader to your neighborhood describing the people, the places, your likes and dislikes about the community, and how long youve lived there. Include one photo from Stage I.
Page 2: Introduce the concept of the Social Determinants of Health. Include a clear definition in your own words. Name the 5 CDC SDOH categories (from text and class) and close by stating this paper will focus on the Neighborhood and Built Environment and provide examples (Access to Foods, Environmental Conditions).
Pages 3 & 4: insert photos from Stage I photos of the SDOH health in the community and descriptions
Pages 5 & 6: Insert Stage II reflection of the Social Policy in Concrete video.
Page 7: Insert Stage III Shop Healthy Assessment.
Page 8: Close with your thoughts on what efforts could be made to educate and promote health in your neighborhood based upon what is being done in Oakland and Detroit and other ideas covered in class. Close with photograph from Stage I that represents something you like or is positive about your neighborhood.
