This week, we have been focusing on collaboration/relationships (Chapter 9) and motivation (Chapter 10). The case you are reading this week is a tragic example of what happens when collaboration breaks down, and is an example of the gravest error possible in healthcare. Your case focuses on the story of 5-year-old Matthew Siravo. He died while in the care of Childrens Hospital Boston after having an elective neurosurgery to attend to a seizure condition he had. Although the surgery was elective, as noted in the case, the procedure was still quite serious. It also involved multiple team players: the neurosurgeon, the neurosurgical resident, two epileptologists, the epilepsy fellow, the director of the MSICU (Medical Intensive Care Unit), the MSICU attending physician, the MSICU fellow, the MSICU charge nurse, and two additional nurses from the MSICU. As noted in the article from the School of Public Health at Harvard: According to the report, Siravo died not from medical complications, though his casewas certainly complex, but due to a breakdown in communication, a murky administrative structure, poor project management and a pass-the-buck institutional culture a situation where lines of authority were deeply tangled, and where no one person had accountability for the patient,’ as the Globe put it. Before writing your response, Id encourage you to read the case, as well as the two articles linked above that provide added coverage of the tragic situation.
After you have read the case and the two support articles, your case write-up should answer the following questions (response 1 page maximum, Times New Roman, size 12, single-spaced, 1 margins):
1. Is there a specific instance where you found the collaboration broke down in the case? Can it be blamed on any one person? Yes or no? And, does blaming it on one person help in any way?
2. Who were the employees accountable to? Do you think they had a clear sense of that accountability, or were people on different pages?
3. Put yourself in the position of being in charge of Childrens Hospital Boston. How would you take steps to instill views of confidence in the hospital after this tragic incident? This can involve how you would inspire confidence with outside stakeholders, but also the members of the healthcare organization.
