After completing the readings and viewings, reflect on how technology is changing health and healthcare, and providing additional resources for chronic disease management. Think about some of the challenges and barriers that individuals, and healthcare providers, might face when accessing or using technology. How can technology be used to inform evidence-based practice? Then consider how the nurse can use technology to support patients with chronic diseases as they manage their health. Include reflection and references to the readings in your paper. You may also include personal or professional experience to illustrate your discussion.

Informatics and technology are used in health and healthcare, by individuals and organizations. What is the difference between health informatics, nursing informatics and consumer informatics? Provide examples to illustrate your discussion.
Role of Technology in Chronic Disease Management
In this section of your paper, focus on the role of technology in chronic disease management. Reflect on the potential benefits of different applications and/or platforms described in the readings, considering both the patient level and the provider level. What barriers or challenges might patients face in accessing/using technology to help them manage a chronic disease? Might these be different for different patient populations – who might have greater challenges, and why? What barriers or challenges might providers face when using technology to improve management of their patients’ chronic diseases? How can technology contribute to evidence based practice in healthcare? Be sure to refer to the readings / viewings.
Application to Nursing Practice
Finally, discuss how nurses can apply technology as they work with patients with chronic diseases. How can the nurse better support their patients to achieve optimal levels of health and wellness, using the technological resources available. Provide examples from your practice as appropriate – what could you have done with a specific patient if you had the appropriate resources and skills?
