Assignment I Community Health Proposal

The windshield survey and selection of a community agency were completed in the previous week. This week you will propose a community project focused on improving the health for your selected vulnerable population group. Some examples of community health projects include:

1.Working with staff members/key stakeholders to identify unmet needs, analyze data, develop, or implement a new program, intervention, or policy.
2.Organize or participate in a health fair.
3.Organize or participate in a screening, immunization, or outreach program.
4.Work with agency volunteers or staff to identify and develop a plan to address needs, gaps in care or services.
Make sure to visit your community agency and log your hours in the practice experience log. Consult with the staff and seek information on what has been done previously. Ask the staff what recommendations they have to improve the health of your selected vulnerable population.
You will write an APA style paper for your proposal. The following are the criteria for the paper.
1. Summarize the proposed project
A. Need and supporting data.
B. Description of vulnerable population to be served.
2. Describe the community agency (EP-SLO#2)
A. Agency overview
B. Organization history
C. Mission and goals, future challenges & long-range plans
D. Current programs and activities
E. How the agency promotes safety, legal, ethical, and moral standards within the community.
3. Describe the health issue
A. Description of issue or need to be addressed.
B. How does addressing the health issue promote health?
C. Supporting facts and documentation of need for the proposed project.
4. Describe the project
A. Description of proposed program
B. Identify target population and number of people to be served.
C. Description of project goals and activities
D. Timetable for implementation (Must be completed by Week 4)
5. Evaluate – What impact will you have? How will you know? Who will do the evaluation? How will the results be used?
The paper must adhere to the following criteria:
The cover page must include the following:
Title of the paper
The students name.
Course number and name
Instructors name
Date submitted.
The assignment must have a minimum of two references.
All references must be cited within the text and all in-text citations must have a reference.
The paper must be written in APA format using 7th edition.
The paper must have an introduction and a conclusion.
The paper must have a separate references page formatted in APA style.
The paper must be a minimum of four double-spaced pages, which does not include the cover and reference pages.
