Develop a evaluation plan in narrative form that is designed to address formative (process) and impact (outcome) evaluation of the program plan that you were assigned. Use the logic model that you have been provided to develop evaluation questions to address, and the approach to be used to assess both the process of executing the plan as well as the preliminary impact of the program on 2 outcomes.
The final program plan should be limited to eight pages and table of contents. You can include an appendix, if needed. The final evaluation plan should include the following key sections:
Introduction: State the purpose of the report. Briefly introduce the public health initiative selected for evaluation and the purpose of the report.
Background and Approach to the Evaluation: Describe the public health initiative more fully, including the goals, objectives, strategies and activities. Address who the stakeholders will be in the process.
Evaluation Study Design: State the evaluation questions. Describe the process and impact evaluation plans.
Evaluation Methods: Describe in detail the data collection and data analysis methods for each aspect of the evaluation
IRB Considerations: Describe how the plan addresses the ethical values of privacy, respect, and beneficence
Dissemination Plans: Describe how you plan to share this information.
