Instead of hourly skin checks, I will implement TWO (2) SKIN CHECKS FOR THE STAFF NURSES per 12-hour shift. The staff nurses will be asked to complete the TWO (2) SKIN CHECKS AT LEAST 6 HOURS APART, —INITIAL SKIN CHECK BEFORE 10am WITH THE SECOND SKIN CHECK BEING COMPLETED AFTER 4pm. Patient Care Assistants should report to nurses, any signs and symptoms of pressure wounds during their typical daily duties.
THE STAFF NURSES WILL PUT IN A REFERRAL TO OT/PT FOR AN EVALUATION OF THE NEED FOR PRESSURE RELIEF DEVICES. OT/PT WILL PROVIDE THE PRESSURE RELIEF DEVICES, THUS RELIEVING THE NURSES OF THIS RESPONSIBILITY.

For instance: In our example, the information necessary to determine project outcome achievement is the incidence of CAUTI on the unit before and after the project was initiated.

For instance: In our example, the data collection method would be tracking/monitoring the CAUTI incidence on the unit. Collaboration with the unit manager may be necessary to obtain this data.
