Environment Exposure Assessment
Assignment overview:
In this assignment, students are to perform a patient interview and complete an exposure assessment on an individual using Table 19.3 (pg. 475) as a guide. Keep the interview focused on environmental factors that could affect the individual’s health and redirect the individual if they begin to discuss other aspects of health such as diabetes, hypertension, or psychosocial factors.
Students are expected to provide a succinct overview of the individual’s responses, and include subjective and objective data obtained during the interview for present and past work/employment, home/residence condition, and activities/hobbies.
Students are to use the data obtained from the individual to identify environmental exposure concerns, recommended interventions, and resources. Use your text and/or additional resources to assist you with identification of actual or potential environmental exposures.
Recommendations to address environment exposures are not to be based on opinion, rather include evidence-based practice (EBP) guidelines that can be found in your text, journals, healthcare website (government and non-government sites), and health policy papers.
Resources used to develop patient recommendations must be cited in the recommendations sections and be included in the reference list.
Recommendations should be shared with the individual being interviewed, and their response(s) should be included in this assessment.

Current or previous homes/residences
Current or previous activities/hobbies
Identified concerns related to exposure history
Recommended interventions and resources that may assist the individual
Assignment formatting:
1. Students are to use APA formatting (professional paper format not student paper format) and include citations and references to support insights, concerns, and recommendations.
2. Citation of personal communication within an interview can be found in the APA manual and is required in this assignment.
3. Responses are to be
written out using professional grammar and accurate spelling.
4. Students are to include a title page and reference page (no abstract required).
5. The length of the interview will be dependent upon the amount of information offered by the interviewee. Some patients may have lengthy exposure histories while others may not. Therefore, a page length is not required, but a well-developed and documented interview is required.
INTERVIEW
*Present work/employment: Currently employed at Stericycle. Works on the dock, unloading empty secured hazardous containers off the trucks. Possibly Exposed to chemicals and bodily fluids. Wears gloves and steel toe shoes for Personal protective equipment. Does not wear clothing from work home, changes in the locker room.
*Past employment: Auto glass agent. Booked appointments and restocked auto glass inventory via telephone from manufacturers. Current employment is longest job held. Not exposed to anything hazardous in previous job.
*Home/residence: Currently resides in a three unit apartment building on the third floor. Year establishment was built is unknown. No recent renovations to the unit. Gas heat throughout the unit. Drinking water is supplied through the city. Regular household cleaning products are used: bleach, dishwashing liquid, Lysol bathroom spray, and Ajax. Past residence was in grandparents home. House was built in 1920. No lead paint knowledge. No mildew, mold, or water damage to property chemicals used in yard maintenance unknown. Household cleaning products used: Lysol, bleach, Ajax, dishwashing liquid. Gas heat in the home.
*Activities/hobbies: basketball and gaming. Does not hunt, fish, burn or melt anything, or grow own foods. No traditional or cultural practices.
*No concerns about current or past exposures. Does not feel like health is being affected by current or past exposures. Never felt sick after coming into contact with any chemicals or substances at home or work. Not experiencing any symptoms at all.
*Identified concerns: possible exposure to chemicals or bodily fluids, disease due to close quarter living (subject said before moving in apartment there was a water bug infestation in apartment unit. Management had to fumigate. There was also a rodent problem and subject got a cat to control the mice) and work environment.
