The purpose of this assignment is to gain insight into the components and application of theory in community prevention programming. Over the past few weeks you have been piecing together and revising your community intervention report. For the final portion of this assignment, you will finalize your write-up. See below for detailed instructions and formatting.
To Complete this assignment:
Finalize your Community Intervention Report write-up with the following:
Section 1 (Intro/Background and Behavior change theories)
Draft section 1 of the report, which provides more detailed information that you have gathered about the selected health topic. The following components are to be included:
Introduction and Background to the topic
Prevalence of the issue
Does the health condition disproportionately impact certain populations/groups?
Behavior change theories (identified based on information gathered in class) that have been used or could be used to provide direction for programs and services that pertain to the topic
Submit a draft of section 1 to this assignment area by the beginning of Week 3.
Feedback and guidance will be provided by the instructor. This feedback is intended to improve your performance on the final assignment.
Section 2 (NGO/GO Interview reflection)
On the same document you turned in for section 1, summarize what you learned from the NGO/GO interview
What theory or theories and related constructs (pieces and parts that make up the theory) did the program adopt?

What evidence have they gathered and analyzed regarding their programs effect? (what outcome are they desiring to impact?)

FORMAT AND MECHANICS
Assignments must be typed, double-spaced, using 12 point Calibri or Times New Roman font. Your assignment needs to be in a Word document. Please number all of your pages and place your name as a header on each page. All pages must use 1 inch margins.
Use a running header
Intervention reports should not exceed 7 pages (not including Appendix)- so please make sure that you are thorough but concise.
Include an appendix that contains a transcript of questions and answers conducted during the interview
Provide contact information for the person that was interviewed (Full name, email, and phone number). This contact information can be included in the transcript
References used for the assignment must be formatted in APA style and listed at the end of the assignment under the heading of References.
