Community Action Plan
Using the Windshield Survey and Epidemiology assignments create a Community Action plan to address one issue identified in the community. Use the research from the Epidemiology assignment to determine an Evidence-Based solution to the issue. Identify the following in your paper:
Assessment: The focus in this phase is on data collection, analysis, dissemination and management.

Policy development: The focus in this phase is planning, analyzing and formulating policy, establishing legal authority, and implementing and evaluating policy.
o Summarize your problem in one statement. List your expected outcomes and the strategies you will utilize to achieve these outcomes. Prioritize your strategies. Include cost of implementing the strategy. Identify the legal policies and procedures that may affect this project, and how you will manage their effects. For example, will you need new policies or procedures? If so, describe them and how you will achieve the necessary revisions. Describe your plan for educating the appropriate populations about the overall plan.
Assurance: The focus in this phase is accomplishing the work. It requires good management of resources and strong leadership.
o Develop a time frame for your strategies. Who is responsible for each step? Suggest a possible situation that may interfere with the achievement of your goal. How will you manage this? What particular leadership and management skills will you need?
o Describe the type of community advocacy that would be needed by you, the nurse, to make this project a reality.
o Who are the stakeholders that could help further this project by helping you get it off the ground?
o Which legislators might be able to help you make your project a reality?
