The two assessment forms below, describe the family using the rubric as your guide including environment, background history, family developmental stage and tasks, family structure and function of family, and healthcare beliefs and practices as outlined in the rubric. You do not have to answer all the questions on the assessment tools – these are a guideline to help you. If you choose a movie, the family or families in the movie provide you with a family in which to do your assessment on.
