This remediation is due to behind in clinical hours. NOT papers written. Professor wants a remediation due to the getting behind in clinical hours due to COVID sickness and my preceptor out of office. Do not understand this assignment much just going to send the downloaded instructions. My Project change proposal is based in a Rehabilitation hospital that specializes in Rehab for stroke, heart attacks, amputations, trauma events. The patient population i am doing the change proposal on is low income, homeless, no income, mentally disabled, domestic violence victims, etc. I will be making a flyer for these patients at discharge to take with them that list the resources available for them to get on their feet quicker and this outcome of this will be less readmissions to hospitals due to decreased poor environments with these patients.
