Scholarly resources are defined as your textbook, books via the AWC library, or peer-review journal articles.
Within your paper address the following components:
An Assessment of the client (hint: the main character in the movie): A detailed account of the client that provides a holistic view of the client and the problems with which they present.
Identifying information: Include the client’s name, gender, date of birth, occupation, and marital status (if this information is not provided write unknown).
Reason for Referral: Why based on the film should or is this person being referred for treatment.
Presenting Problems and Chief Complaints: What are the client’s problems and how long have they been experiencing the issue(s). Also, describe if applicable what the client has done in the past to try to resolve the issue.
Client History and Relevant Background Information:
If applicable include:
The client’s current general health and functioning. Mention any history of the disease, accidents, and medication (remember phineas gage and how important this information can be with presenting problems or changes in moods).
The client’s mental health history. Mention any history of abuse, violence or neglect.
The client’s current employment status and occupation, an overview of their work history, and any special skills or training they have.
If the client was or wasn’t in the military. If they served in the military, describe their role, the dates they served, any highly stressful experiences they had, and the type of discharge they received.
The client’s strengths and resources: Evaluate how your client copes with issues. Mention their strengths and limitations.
Any legal concerns your client has or dealt with in the past, including marital issues or domestic violence.
The client’s current functioning level and if they can meet their basic life needs, such as obtaining housing and food. Write if they require assistance.
The client’s substance abuse background, if any. List current and past experiences, such as when they started using drugs or alcohol, and how much they used.
The client’s family members, including details about the client’s childhood relationships and who they grew up with. List family members’ history of substance abuse, legal problems, or psychiatric disorders.
Assessment Summary:
Brief summary: In about five sentences, summarize what you’ve already written in your assessment. Your summary should include the presenting problem your client has and the contributing factors. Mention any secondary issues or concerns you may have.
Client Diagnosis (Please utilize the DSM V and Text for diagnostic criteria). Include a rationale for your diagnosis based on presenting problems.
Treatment Recommendation(s):
Identify the name of at least one theory on which you base your treatment plan and what skills are intended for use with explanations (ex: cognitive restructuring, dream analysis, or client-therapist relationship features).
Identify if medication management is an option for this client and the type of medication the client should receive based.
