As you’ll recall from the Syllabus, you will be required to write a 2-3 page topic paper on a significant safety event or subject, such as an accident, an effective safety concept or procedure.
Reports on accidents should include probable causes, how the accident could have been avoided, and the error chain. Concepts or procedures should be illustrated with examples from the history of safety investigations. Your paper may include pictures, graphics or data if it helps to illustrate the theme.
Topics are subject to approval of the instructor (me). You should select a topic early enough in the term that you can research and write it well before it is due.
Four of you have already chosen your topics, and I have approved them. The rest of you need to choose your topic in the Discussion Forum ASAP. If you don’t have anything specific incident in mind yet, touch base with me for some suggestions.
Note that you may not write about the same topic that another student has chosen. To prevent that, the first student to post gets to write on that topic. So, read all the previously-posted topics before you submit your own. You may not write about accidents that have been covered in the course. That will still, unfortunately, still leave you plenty of mishaps to choose from.
Full APA-style citations are not required, but you need to list your references. You may use APA style if you wish, but, at a minimum, the citations must be specific enough that, if I wanted to, I could look up where you found the information and review it myself.
