Identify a specific patient population within a community of interest. Students will collaborate with their Inter-professional Collaborator (IPC) to identify their selected community of interest and to complete the Community Needs Assessment. Additional collaboration will occur between the student and their select preceptor to develop a community action plan, create a community action plan intervention, and complete a community action plan presentation.
