A copy of your previously submitted lesson plan (make sure this includes revisions if applicable adjusting feedback was given)
A copy of student work (1-2 examples)
An answer key with exemplar responses
A copy of the assessment used at the end of the lesson to measure whether or not students mastered the objective
A copy of any other reference materials (anchor charts, etc.)
A 1-2 page reflection with follow-up goals regarding the implementation of these strategies (post-lesson).
