Logic Model
Download a copy of the logic model introduced in Lesson 8.1 by selecting the download icon, , next to the tables title. Fill out the table with information related to your own DNP project. Consider reviewing the Hayes et al. (2011) reading that was assigned for this week, the source of this logic model, so you have a clear and completed example for reference as you work on your own table.
Narrative Evaluation Plan
Draft a 500-word narrative explaining your evaluation plan. Consider starting from the logic model that you completed in Step 1: Describe, elaborate, and justify the information that you entered there. Be certain to consider this weeks objectives and include suggested evaluation elements from your EBP change model of choice, including measures to evaluate your AIMS and the specific statistical tests that you will use.
The EBP model that I chose was the Havelock’s. I have attached the PowerPoint with my Aims and objectives of my project. The PICOT Question is:
