Students are encouraged to reference an older adult client case from field practicum if possible (de-identified for confidentiality). If the student does not have an actual client to base this assignment on, you may use an older adult family member or friend (de-identify for confidentiality). The case conceptualization form must be completed fully with as much detail as possible and include a biopsychosocial-spiritual assessment, differential diagnoses, and treatment plan with a specific intervention description. Outside scholarly articles and course texts must be cited. Use APA 7 format.

Client is a. 62 y/r old lady who lives alone. Client has two sons one died when he was only 24 ys old. The other son lives in Washington with wife and two grandsons. Client has spina bifida, arhtritis, fybroyalgia. patient is on section 8 for the past 21 years. Patient has two caregivers twice a week.
