Definition of the Problem: At the Orthopedic Hospital patients receive a dose of antibiotics before surgery and then not again until after surgery, some not even at all are redosed. With the rise of surgical site infections, at look into the antibiotic protocol should be done to determine if administering more doses of intraoperative antibiotics will decrease infection rates.

Development of Solution Options: Decreasing surgical site infections with the redosing of antibiotics would be ideal. In order to determine if the redosing of antibiotics intraoperatively would improve patient outcomes, a study would have to be conducted to determine if administering more antibiotics during the perioperative phase would decrease infection rates. The physicians and hospital would have to be willing to be a part of the research of redosing of antibiotics throughout the intraoperative phase.

Feasibility of Planned Change: The patients will have to have orders in place to have a preop antibiotic dose, then a regimented dosing plan to redose the antibiotics in a predetermined time frame, such as a dose before incision, then every 1 or 2 hours after initial incision, then another dose postoperatively. The physicians will have to be willing to prescribe the dosing to the standing order set. This will then have to be executed by the preop nurse, circulating nurse, and recovery nurse. The dosing regimen will have to be adhered to and documented so the data for the patients would be the same as far as timeline of antibiotics given during the perioperative phase. The cost of the research would be the cost of the antibiotics for the patient or if the hospital would be willing to try it on a trial basis and not charge the patient for the antibiotics. The hospital would then be out the cost of the antibiotics. In order for the plan to be successful there would need to be education to nursing staff as well as physicians that would include the purpose of the trial and what is the goal of the trial when it is complete, as well as defining the purpose, which is to ultimately decrease surgical infection rates of patients at the hospital overall.

Problem Statement: A study done to determine if redosing of antibiotics to surgical patients during the intraoperative phase would reduce the rate of surgical site infections. The study would focus on a specific group of patients with a clear order set of antibiotics and a pre-determined dosing pattern for the antibiotics. An examination of the number of surgical site infections from previous 90 days to current 90 days to determine if the antibiotics reduce the rate of infection. GOAL: To decrease surgical site infections by standardizing the redosing of intraoperative antibiotics to every 2 hours after initial incision by June 1, 2023.
